Job summary

Medical Services of America’s Medi Home Health Agency in Pittsburgh, PA is seeking a dedicated Registered Nurse (RN) for full-time home health care. You will provide one-on-one skilled nursing services, assess patients, and coordinate with physicians to create individualized care plans.

The role requires an active PA RN license and at least 1 year of nursing experience, preferably in home health. Competitive hourly pay and benefits accompany a patient-centered care approach.

Qualifications

• Completion of an accredited nursing program.
• Active Registered Nurse license in the state of Pennsylvania.
• At least 1 year of clinical nursing experience (Home Health strongly preferred).

Responsibilities

• Provide one-on-one skilled nursing care to home health patients.
• Perform comprehensive initial assessments and develop individualized care plans in coordination with physicians.
• Monitor, evaluate, and document patient progress and adjust care plans as needed.
• Deliver education and support to patients and families to empower them in managing care at home.
• Ensure all care provided aligns with agency policies, clinical guidelines, and physician orders.
• Participate in rotating on-call responsibilities to ensure continuity of care.

We’re currently seeking a dedicated and experienced Full-Time Registered Nurse (RN) to join our growing team and provide skilled, patient-centered care to individuals in the Pittsburgh (Allegheny County), PA area.

What You’ll Do:
