Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) for full-time or part-time home health visits serving adult patients in Smyrna, Clayton, Dover, DE. You will be part of a multidisciplinary team delivering rehabilitative care and skilled nursing in the home.

Responsibilities include following care plans, administering medications, wound care, and documenting care using EMR. Travel to assigned cases is required; strong communication and time management are essential.

Qualifications

• A current RN license in DE is required.
• Minimum of one year of recent, verifiable clinical experience (medical/surgical).
• Home care experience is preferred but not required.
• Graduation from an accredited nursing program.
• Ability to work independently and manage time effectively.
• Strong interpersonal and communication skills; proficient EMR use.

Responsibilities

• Follow a designated care plan based on patient needs.
• Make home visits to clients in assigned geographic territories.
• Administer medications, provide wound care, and perform treatments.
• Monitor patients' conditions and report changes to the Clinical or Client Services Manager.
• Process and document doctors' orders accurately and promptly.
• Conduct client assessments as needed and support case management.

BAYADA at BAYhealth is currently seeking a Registered Nurse, for a Full Time or Part Time opportunity performing home health visits with adult patients. This office services clients throughout Smyrna, Clayton, Dover, DE.

As a home care Registered Nurse, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.

Registered Nurse (RN) Responsibilities:

• Follow a designated care plan in accordance with patient's needs
• Make home visits to clients in designated geographic territories
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures
• Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
• Follow up with, execute, and properly document doctors' orders
• Perform client assessments as necessary
• Case management and coordination
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet

Qualifications for a Registered Nurse (RN):

• A current license as a Registered Nurse in DE.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience strongly preferred, but not required.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.

Additional Information:

Base Pay, depending upon qualifications, status, and subject to negotiation:

$62 - $67 per point ($96,720 - $104,520 annual earning potential)
