Job summary
AccentCare is seeking an RN Case Manager for Home Health at our Petaluma Office. This full-time role covers American Canyon, Napa, Rutherford, Calistoga with a Monday–Friday 8 am–5 pm schedule and a pay-per-point compensation plan.
You will need an RN license in the state, CPR certification, a valid driver’s license and reliable vehicle, and at least one year of RN experience. AccentCare offers medical, dental, vision, PTO, 401(k) and more.
Qualifications
- Graduate from an approved school of professional nursing and licensed in the state of operation.
- Minimum of 1 year RN experience.
- Must have CPR certification.

If you meet these qualifications, we want to meet you!
- Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
- One (1) year experience as a RN.
- Licensed to practice as a registered nurse in the state of agency operation.
- Must possess and maintain valid CPR certification while employed in a clinical role.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Ability to travel to all business locations.
