Job summary

AccentCare is seeking an RN Case Manager for Home Health to join our team in Tennessee. This full-time, on-site role covers Shelby, Tipton and Fayette Counties with a travel component to patient locations.

You will coordinate patient care, collaborate with clinicians, and ensure quality outcomes in a home health setting. The ideal candidate holds an approved nursing degree, an active TN RN license, CPR certification, and at least one year of RN experience; a valid driver’s license with

RN / Registered Nurse, Home Health

Location: Memphis, TN

Position: RN Case Manager, Home Health

Position Type: Full-Time

Remote/Virtual Position: No

Coverage Area: Shelby, Tipton and Fayette Counties, TN

Hourly: $32.00-$38.00

Schedule: M-F 8a-4:30pm (on-call)

Payment: Per-point basis; compensation estimate hourly

Qualifications

• Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
• One (1) year experience as an RN.

Required Certifications and Licensures

• Licensed to practice as a registered nurse in the state of agency operation.
• Must possess and maintain valid CPR certification while employed in a clinical role.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Ability to travel to all business locations.
