Job summary

Interim HealthCare of Augusta is seeking an experienced RN to join our home health team, delivering personalized nursing care in patients' homes across Augusta and surrounding communities.

As an RN in home health, you will perform assessments, implement an individual plan of care, administer treatments, educate families, and document all clinical activities, while collaborating with a supportive healthcare team.

Qualifications

• Current and unrestricted Registered Nurse (RN) license.
• Current CPR/BLS certification.
• Strong clinical assessment and critical-thinking skills.
• Excellent communication and documentation skills.
• Reliable transportation and a valid driver’s license.
• Dependable, professional, and compassionate.
• Ability to work independently in a home care environment.
• Home health experience is a PLUS!

Responsibilities

• Perform comprehensive patient assessments and monitor changes in condition
• Provide skilled nursing care according to the patient’s individualized plan of care
• Administer medications and treatments as ordered
• Provide wound care and other skilled nursing services as needed
• Educate patients and families on medications, treatments, and ongoing care
• Communicate with physicians, caregivers, and other members of the healthcare team
• Maintain accurate and timely clinical documentation
• Recognize and report changes in a patient’s condition
• Provide compassionate, patient-centered care in the home setting
