Munson Healthcare in Michigan is seeking Home Health Registered Nurses to provide skilled nursing care in patients’ homes. This role offers no on-call shifts and a weekend rotation of just one weekend every four weeks, with flexible scheduling to balance life and work.

Qualifications

• Licensed RN in the State of Michigan.
• Experience in acute care, home care, or hospice preferred.
• Strong health assessment, organizational, and interpersonal skills with the ability to make independent judgments directing patient care.
• Able to meet Home Health and Medicare standards through criminal background check and fingerprinting.
• Must have current Michigan driver’s license and a car with insurance for patient visits in varying weather.

Responsibilities

• Manages an assigned caseload from admission through discharge using assessment, planning, coordination, and evaluation.
• Completes comprehensive assessments (including OASIS when required) and develops/updates individualized plans of care with skilled nursing services across preventive, rehabilitative, palliative, and end-of-life needs.
• Educates and counsels patients and families on disease processes, symptom management, and self-care; coordinates care with interdisciplinary team members.
• Supervises LPNs and paraprofessionals; communicates changes in patient condition to physicians and team members; ensures equipment and supplies; maintains thorough documentation in the EMR.

Job Description

A Day in the Life

• Manages an assigned caseload from admission through discharge using a framework of assessment, planning, coordination, and evaluation.
• Responsibilities include completing comprehensive assessments (including OASIS when required), developing and updating individualized plans of care, and providing skilled nursing services across preventative, rehabilitative, palliative, and end‑of‑life needs.
• Educates and counsels patients and families on disease processes, symptom management, and self‑care, and coordinates care with interdisciplinary team members such as Home Health Aides, Therapies, Social Work, Chaplaincy, and Bereavement Services.
• This position also supervises LPNs and paraprofessionals, communicates changes in patient condition to physicians and team members, secures necessary equipment and supplies, and maintains thorough, timely documentation within the electronic medical record.

Qualifications

What’s Required

• Licensed RN in the State of Michigan
• Experience in acute care, home care, or hospice preferred
• Proven health assessment, organizational, and interpersonal skills with the ability to make independent judgments directing patient care.
• Able to meet Home Health and Medicare standards through criminal background check and fingerprinting.
• Possess current Michigan motor vehicle license, ability to drive a car and an insured vehicle capable of transport to patient homes or other offices in various weather conditions.

Additional Information

Munson Healthcare requires all employees be vaccinated or have lab confirmed immunity for Measles, Mumps, Rubella and Varicella. MHC also requires all employees to receive a flu vaccine during the flu season in the year that they are hired and annually thereafter, or receive an approved medical or religious exemption.
