Qualifications

• Associate degree in Nursing or graduation from an accredited school of nursing and RN license.
• 1 year related experience or training.
• AHA BLS Healthcare Provider certification; RN - NJ.
• Requires ATCN or TNCC and 8 hours trauma CNE annually if ICU/ED assigned.
• Requires NIH Stroke Scale certification and 8 hours stroke CNE if assigned to related units.
• Requires ACLS within 6 months if assigned to critical care or ED settings.
• Requires NRP within 6 months if assigned to maternity services.
• Requires PALS within 6 months if assigned to pediatric areas.

Serves as an advocate by providing emotional and informational support to patients and families and giving them an opportunity to participate in goal setting as they consider alternatives to care.

MINIMUM REQUIREMENTS

• Education: Associates degree or graduation from an accredited school of nursing. Registered Nurse.
• Experience: One year related experience or training.
• Other Credentials: AHA BLS - Healthcare Provider, Registered Nurse - NJ
• Requires ATCN or TNCC (or must obtain within 1 year of hire) AND 8 hours of Trauma related Continuing Nursing Education (CNE) contact hours annually if assigned to: RMC ICU/CCU, RMC Surgical Trauma Unit, RMC PACU, Emergency Department (all campuses)
• Requires NIH Stroke Scale Certification (or must obtain by end of orientation) AND 8 hours of Stroke related Continuing Nursing Education (CNE) contact hours annually if assigned to: Critical Care, Intermediate Care Unit, Emergency Department, Neuro Units, Cardiology Inpatient at Hopewell, Peds ED, PACU, Interventional Radiology, CNI, Observation, Hospital at Home
• Requires ACLS (or must obtain within 6 months of hire date) if assigned to: Critical Care/Intermediate/Telemetry, Emergency Rooms, Pediatrics/Pediatrics Emergency Room, Labor & Delivery, Surgical Services (not to include Perioperative), Interventional Procedures, Observation.
• Requires NRP (or must obtain within 6 months of hire date) if assigned to: Maternity Services
• Requires PALS (or must obtain within 6 months of hire date) if assigned to: Emergency Rooms, Infant Follow-Up, Surgical Services (only PACU & Same Day Surgery), Pediatrics/Pediatric ED.

ESSENTIAL FUNCTIONS

• Plans for providing traditional and individualized comfort measures. Fosters an environment to include the core mission. Serves as an advocate by providing emotional and informational support to patients and families and giving them an opportunity to participate in goal setting as they consider alternatives to care.
• Provides appropriate support for pain management, pharmacological and non-pharmacological measures. Assist patient/family in identifying individualized comfort measures.
• Initiates plan of care based on Capital Health core mission principles. Includes illness, pain, fear, anxiety and/or any other stressful emotion in plan of care. Advocates for patient when condition prohibits self-advocacy. Incorporates goal setting and consideration of alternatives in the development of a viable plan of care.
• Identifies the patient's readiness to learn, formulates and implements individualized teaching plans, documents outcomes. Provides interpretation of the patient's condition and gives rationales for procedures which are consistent with the health team.
• Plans for and documents discharge needs of the patient with appropriate assistance.
• Determines multiple teaching needs related to identified patient problems and documents patient readiness to learn. Provides consistently information to patients with validation through documentation, observation and staff input. Provides patient with information to meet educational needs at time of discharge.
• Identifies and documents the patient's initial condition and need for care based on a detailed assessment. Interprets and assesses changes in patient condition, plans the appropriate intervention to achieve expected outcomes and documents changes, interventions and outcomes.
