Job summary

HealthPartners is seeking a Registered Nurse Float for Urgent Care. The role will float to multiple clinics including Woodbury, Arden Hills, Apple Valley, St. Paul Como and St. Paul Wabasha, with further sites added in the future.

Responsibilities include providing comprehensive nursing assessments, planning and implementing care, delegating tasks, and educating patients and families. Collaboration with multidisciplinary teams is essential to deliver high-quality care.

Qualifications

  • Graduate of an accredited school of nursing.
  • Current RN licensure in Minnesota.
  • Current RN licensure in Wisconsin within two months of hire.
  • Current BLS certification for health professionals through the American Heart Association or Red Cross required and maintained. Online BLS certification is not acceptable.
  • Strongly demonstrated written and oral communication skills.

Responsibilities

  • Provide high quality nursing care to patients and their families.
  • Assess, plan, implement and evaluate patient care needs.
  • Delegate nursing tasks when appropriate and supervise staff.
  • Create and implement teaching plans based on identified patient needs.
  • Act as a resource for other staff and coordinate care across settings.
  • Engage in care coordination to ensure continuity of care across home, office, inpatient and community settings.

QUALIFICATIONS
REQUIRED

Graduate of an accredited school of nursing

Current RN licensure in Minnesota

Current RN licensure in Wisconsin within two months of hire

Strongly demonstrated written and oral communication skills

POSITION PURPOSE

Nursing Process

Definition

Definition:

Uses health coaching techniques to elicit patient goals and priorities to develop individualized patient action plans

Uses population management tools such as registries to identify high risk patients and track, monitor and follow-up with their care

Engages, assesses readiness to learn and educates patients and families to implement strategies to reach a desired goal or care intervention

Identifies and secures patient services needed for patient care

Informs and supports patients and families to assist them in making informed decisions and navigate the healthcare system.

Education

Definition:

Education imparts knowledge designed to develop skills, to enhance decision-making, and to promote understanding. It includes interaction with the patient, family and community, as well as participating in formal and informal professional education.

Works in partnership with patient/family to identify patient and family needs and develop a plan for education and/or make appropriate nursing referrals.

Teaches techniques or knowledge necessary for prevention, self-care management or rehabilitation.
