Job summary

Bluegrass Care Navigators is seeking a Flex/After-Hours Hospice RN to provide compassionate, patient-centered care to individuals in home and facility settings, including personal residences, nursing facilities, and hospitals. This hybrid role supports care outside regular hours and adapts to evolving hospice team needs.

You will work with an interdisciplinary team, communicating clearly, maintaining professionalism, and upholding hospice philosophy.

Qualifications

• Graduate of an accredited school of registered nursing.
• Currently licensed as a Registered Nurse (KY BO Nursing or Compact State Board).
• Minimum of one year of general nursing experience preferred; new graduates considered.
• Demonstrates strong clinical nursing skills and the ability to maintain professionalism in emotionally challenging situations.
• Provides compassionate, patient-centered care while serving diverse populations with respect and cultural sensitivity.
• Adheres to professional, ethical, and legal standards of nursing practice.
• Demonstrates understanding of and commitment to hospice philosophy and the principles of death and dying.
• Maintains current CPR certification.
• Must be a licensed driver with an automobile in good working order, insured as per state minimums.

Responsibilities

• Deliver compassionate, patient-centered care in home care, nursing facilities, and hospitals.
• Support continuity of care outside regular business hours with flexible dayshift coverage.
• Respond to urgent patient needs, manage symptoms, support new admissions, and assist with ongoing case management.
• Work autonomously within an interdisciplinary team and communicate effectively with patients, families, and providers.

Job description

As a Bluegrass Care Navigators' Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.

Schedule:

Every other weekend 8:00am-9:00pm, 4 other shifts 9:00am-10:00pm - to be scheduled with manager

Incentive:

$4.00 per hour on-call shift differential pay for shifts after 5:00pm weeknights and all day for weekend shifts, in addition to base pay
