Job summary

LiveWell Homecare Agency seeks an Inpatient Registered Nurse to assess, plan, deliver, and evaluate patient care, ensuring safety, satisfaction, and optimal health outcomes. You will work with interdisciplinary teams to develop individualized care plans and provide direct care, including treatments, medications, and diagnostics.

You will address physical, psychological, social, cultural, and spiritual needs, coordinate discharge planning, educate patients, and continually improve interventions

Qualifications

• Graduate from a Specialty Training Program (required).
• Bachelor’s Degree in Nursing preferred.

Responsibilities

• Perform comprehensive assessments and develop individualized care plans in collaboration with interdisciplinary teams.
• Provide direct patient care including treatments, medications, and diagnostic procedures.
• Address physical, psychological, social, cultural, and spiritual needs of patients and families.
• Coordinate discharge planning and patient education to promote recovery and safety.
• Evaluate effectiveness of interventions and revise care plans accordingly.
• Collaborate with physicians and staff to resolve patient needs.
• Participate in unit-based decision‑making, safety programs, and quality improvement initiatives.
• Troubleshoot equipment issues and uphold evidence-based practices.
