Job summary

Trinity Health Mid-Atlantic is seeking a Crisis Nurse to support individuals in crisis across multiple programs, delivering triage, assessment, and tailored nursing care to promote stabilization and recovery. The role emphasizes safe, person-centered care within a community health setting.

Responsibilities include accurate documentation in EMR, managing med administration including long-acting injections, and coordinating with physicians, pharmacists, and care teams.

Qualifications

  • Bachelor of Science in nursing with at least 1 year behavioral health care experience, or RN with at least 2 years behavioral health care experience.
  • Current RN license in Pennsylvania required.
  • Valid driver’s license required.
  • Pre-employment physical, TB test, drug screening, and Act 33/34/73 clearances.

Responsibilities

  • Conduct medical triage and assessments.
  • Perform case management.
  • Administer medications including long-acting injections.
  • Differentiate emergent/urgent needs from ongoing clinical work.
  • Reconcile medications with physicians and pharmacists.
  • Document in electronic medical records and use various electronic portals.

Employment Overview

Employment Type: Full time Shift: Description: Position Purpose: The Crisis Nurse provides services to persons experiencing crisis and in need of immediate intervention. This position job responsibilities between three programs: Walk-In Crisis Unit, Central Recovery Center (CRC), and Adult Diversion and Stabilization (A-DAS). By assessing, planning, and implementing nursing care tailored to each patient's unique needs, you will contribute to the stabilization and recovery of those in crisis.

What you will do: Completes medical triage and assessment Case Management Medication administration including the administration of long-acting injection medications Being able to distinguish between emergent/urgent needs and ongoing clinical work Medication Reconciliation with physicians and pharmacists. Completes documentation in electronic medical records and utilization of varied electronic portals as needed. Scheduling and tracking consumer appointments Provides medication education

Minimum Qualifications
  • Bachelor of Science Degree in nursing AND a minimum of one (1) year of paid behavioral health care experience OR a Registered Nurse AND a minimum of two (2) years of paid behavioral health care experience.
  • Current RN License from the Commonwealth of Pennsylvania required.
  • Valid Driver’s License required.
  • Pre-employment physical/TB Pre-employment drug screening, Act 33/34/73 clearances.

This is a full-time non-exempt position (37.5 hours/week) Saturday 6:30am to 7:00pm with 2 floating days
