Qualifications

• Active RN license in the state.
• CPR/BLS Certification required.
• Experience in Long Term Care or Skilled Nursing preferred.
• Excellent technical, assessment and documentation skills.
• Strong analytical and problem-solving skills.
• Ability to lift 25+ pounds.
• Ability to stand, bend and walk for extended periods.
• Team player with strong communication.

Responsibilities

• Provide nursing care to meet resident needs per standards and orders.
• Plan nursing interventions based on individual needs and monitor.
• Evaluate care effectiveness and develop alternatives.
• Conduct family and resident education.
• Administer medications and treatments timely per policy.
• Transcribe orders and document progress accurately.
• Identify and report changes in condition to supervisor and family.
• Follow skin management protocols for changes.
• Order and report diagnostic tests timely.
• Coordinate rehab program with therapists.
• Ensure CNA documentation and assignments are on time.
• Perform additional nursing tasks as required by Act.
