CARE PLANNING AND IMPLEMENTATION: Develops an individualized plan with strategies that address diagnoses. Provides for continuity through a plan that directs other members of the team. NEUROSCIENCE RESPONSIBILITIES: Demonstrates competency in assessment, planning, treatment and evaluation of patient experiencing stroke or stroke-like symptoms. Provides appropriate stroke-related education to the patient and/or family/caregiver. Knowledgeable in stroke performance measures and how they relate to patient outcomes. Coordinates multidisciplinary care of the stroke patient. Must have a total of 4 hours of stroke related education annually. Additional Duties Adheres to the hospital and departmental attendance and punctuality guidelines. Performs other duties as required and completes all job functions as per departmental policies and procedures. Maintains current knowledge in present areas of responsibility (i.e., self education, attends ongoing educational programs).
