Job summary

21st Century Home Health Services is seeking a Registered Nurse, Home Health Case Manager, Level 2 to oversee in-home patient care and care coordination across a multidisciplinary team in California. You will prioritize patient needs, lead care planning, and support families and caregivers.

Qualifications

• One year of Home Health Case Management experience preferred.
• Minimum of one year of nursing experience required; two+ years preferred.
• Experience supervising/mentoring nurses highly preferred.

To be a success a Registered Nurse (RN) Home Health Case Manager, Level 2:

• One Year of Home Health Case Management experience is perferred
• Minimum of one year of Nursing Experience required; two+ years preferred.
• Experience supervising, mentoring Nurses is highly preferred
• Current and unrestricted CA RN license
• Current BLS/CPR Certification
• Valid CA Driver's License
• High level of clinical knowledge in home health care is key
• Experience with high acuity patients is a plus
• Experience working with an interdisciplinary team is required
• Case Management fundamentals is required

Target Wage range Registered Nurse, Level 1: 140,000.00- 160,000.00 plus 20,000.00 Annual Bonus, paid quarterly. Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans, comprehensive health benefits and 401k (Up to 4% match).
