Southwest Healthcare Inland Valley Hospital in Wildomar, California, is a licensed 122-bed facility accredited by the Joint Commission. It is Southwest Riverside County's only designated Trauma Center and a county-designated Paramedic Base Station.

Southwest Healthcare Rancho Springs Hospital in Murrieta, California, is a 120-bed acute-care hospital with the region's only Pediatric Emergency Services team and an OB/Emergency Department program providing 24/7 on-site coverage.

Qualifications

• Minimum three (3) years of acute care clinical experience.
• Two (2) years’ experience clinical Case Management or Utilization Review preferred.
• Completion of an accredited Registered Nursing program.

Responsibilities

• Case Managers identify and prioritize the need for further assessment based on clinical presentation and care setting.
• Initiates discharge planning for each patient upon admission and follows the nursing process.
• May float to the other facility within Southwest Health when needed with orientation provided.
