Job summary

Dove Healthcare in Eau Claire, WI seeks a Registered Nurse Case Manager to mentor and educate a team of caregivers while coordinating and overseeing patient care. You will lead and coordinate care plans, collaborate with rehab services, and ensure high-quality, resident-centered care.

You will supervise daily activities, review care plans, manage staffing, participate in on-call rotations, and maintain a safe, home-like environment for residents and families.

Qualifications

• Licensed in Wisconsin as a Registered Nurse (RN).
• Degree from an accredited school of nursing.
• Previous experience in a related field is preferred.
• Ability to prioritize duties in a dynamic environment.
• Demonstrated effective management skills.
• Customer service oriented with patients, families, and colleagues.
• Strong problem solving and time management abilities.
• Ability to take and follow directions; works well in a team.

Responsibilities

• Support rehabilitation services to help residents achieve optimal functional outcomes.

What You’ll Do:

• support and collaborate with rehabilitation services to assist residents in achieving optimal functional outcomes

Schedule:

• Flexible business hours, Monday–Friday


• On-call every 4th weekend and every 4th weekday
