Job summary

TEEMA GROUP is seeking an experienced Registered Nurse (RN) Case Manager to join a collaborative care team serving frail and elderly adults in Carson, CA. The role combines clinic work with home visits in a hybrid setting, managing an assigned patient panel and coordinating with PCPs and an interdisciplinary team to deliver comprehensive, patient-centered care.

You will conduct comprehensive nursing assessments in clinic, virtually, and during home visits; develop and update Plans of Care;

Qualifications

• Graduate of an accredited School of Nursing.
• Active, unrestricted RN license.
• Active BLS certification.
• Minimum 2 years of clinical nursing experience caring for frail or elderly adults.
• Strong clinical assessment and decision-making skills.
• Experience coordinating care for patients with complex healthcare needs.
• Strong organizational, communication, and time-management skills.
• Ability to work independently and make appropriate clinical decisions.
• Comfortable working with patients from diverse cultural and socioeconomic backgrounds.
• Ability and willingness to conduct local home visits.

Responsibilities

• Process and follow up on medical orders, test results, and clinical communications.
• Perform nursing procedures and provide direct patient care when needed.
• Complete accurate and timely documentation in the Electronic Health Record (EHR).
• Conduct local home visits as required.

Pay: $50.00–$54.00 per hour
Schedule: Monday–Friday, 8:00 AM–4:30 PM
Job Type: Full-Time Contract | 13-Week Assignment | 40 Hours/Week
Setting: Hybrid Clinic & Field-Based/Home Visits

We are seeking an experienced and compassionate Registered Nurse (RN) Case Manager to join a collaborative care team serving frail and elderly adults in Carson, CA.

This is an excellent opportunity for an RN with strong clinical assessment and care coordination experience who enjoys developing meaningful relationships with older adults. You will manage an assigned patient panel and work closely with Primary Care Providers and an interdisciplinary team to coordinate comprehensive, patient-centered care.

What You'll Do

Local travel may be required approximately 2–5 times per week for home visits within the surrounding Carson area.
