AltaMed Health Services seeks an RN Case Manager/Care Coordination Team Leader to oversee all PACE participants in the ambulatory setting. You will coordinate comprehensive care, ensure timely evidence-based interventions, and drive quality outcomes within a supportive team.

Minimum requirements include California RN licensure and at least 1 year of geriatric or managed care experience. A current BLS or higher certification is required. Competitive compensation and benefits accompany this role.
