Job summary

Consumer Directed Services Inc. is seeking a field-based clinician to conduct in-home assessments and develop person-centered care plans, coordinating physical, behavioral, and social services.

You will support member transitions, assist with eligibility needs, and navigate coverage options, traveling as required and collaborating with interdisciplinary teams to ensure seamless care.
