Job summary

Health Here in Austin is seeking a Registered Nurse Case Manager to provide coordinated, patient-centered care for diverse populations and to perform comprehensive assessments and develop/update individualized care plans.

You will collaborate with physicians and interdisciplinary teams to ensure safe transitions of care, manage utilization and post-acute services, educate patients, and accurately document in the EMR.

Qualifications
