Job summary

GHR Healthcare is seeking a Registered Nurse Case Manager for a 13-week travel assignment in Charleston, SC. The role focuses on discharge planning, utilization review, and care transitions within an on-site hospital setting.

The ideal candidate has active SC RN eligibility, at least 1 year of nursing experience, and strong communication skills to work with a multidisciplinary team. CCM/ACM certification is preferred but not required.

Qualifications

• Strong knowledge of discharge planning, utilization review, care transitions, and interdisciplinary care coordination.

Responsibilities

• Assist with discharge planning and care coordination.
• Support utilization review and resource utilization decisions.
• Coordinate transitions of care across hospital units and with post-acute providers.
• Collaborate with physicians, nursing staff, social services, patients and families.
• Document assessments and care plans accurately and promptly.
• Participate in weekend/holiday coverage as needed.

Registered Nurse Case Manager | 13-Week Travel RN Contract in Charleston, SC

Join an experienced care coordination team and support discharge planning, utilization review, transitions of care, and interdisciplinary collaboration in an on-site hospital setting.

• Weekly Estimated Pay: $1,577–$1,752
• Location: Charleston area, South Carolina
• Start Date: November 23, 2026
• Assignment Duration: 13 weeks
• Schedule: Monday through Friday, 8:00 a.m.–4:30 p.m.
• Weekend and Holiday Coverage: Weekend rotation and holidays as needed, consistent with departmental scheduling
• Patient Assignment: Approximately 1:25–1:30
• Work Setting: On-site; may float within hospital units as needed

Registered Nurse Case Manager Requirements

• Active Registered Nurse license eligible for practice in South Carolina
• At least 1 year of professional nursing experience
• Previous acute care hospital case management experience required
• Strong knowledge of discharge planning, utilization review, care transitions, and interdisciplinary care coordination
• Excellent communication, organization, patient assessment, and problem-solving skills
• Ability to work independently while collaborating effectively with providers, nursing staff, patients, families, and community partners
• CCM or ACM certification preferred, but not required
• Comfort working with electronic health records, clinical documentation, and care coordination systems
• Business-casual dress is required
• Assess patient needs and coordinate individualized plans of care throughout the hospital stay
• Facilitate safe, timely, and appropriate transitions from acute care to the next level of service
• Collaborate with physicians, nursing teams, social services, patients, families, payers, and post-acute care providers
• Review clinical information to support appropriate resource utilization and continued care planning
• Identify barriers to discharge and develop solutions to promote efficient patient flow
• Arrange follow-up services, durable medical equipment, home health, rehabilitation, and other community resources as appropriate
• Document assessments, interventions, care coordination activities, and discharge plans accurately and promptly
• Participate in weekend and holiday coverage rotations according to department needs
• Maintain compliance with professional nursing standards, organizational policies, and applicable regulatory requirements
