Work schedule: Monday - Friday, occasional Saturday - 8AM - 4:30PM

Accountable for the delivery of quality care to patients and families according to the individualized needs of the patient.

Responsibilities
Essential Job Functions
  • Provides appropriate care specific to the age of the customer/patient, to ensure understanding and comfort level of treatment as outlined in the Population Specific Criteria.
  • Demonstrates ability to identify opportunities for and give individualized customer service/patient care according to customer/patient needs.
  • Performs an initial and ongoing assessment to determine the patient's needs.
  • Includes data from the patient/significant other(s) in the assessment as appropriate.
  • Prioritizes data collection according to patient's immediate needs.
  • Initiate discharge planning upon admission.
  • Comprehensively assesses each patient/family the day of admission (or next day if admitted on off-shifts or Monday a.m. if admitted on the weekend) for medical, psychosocial, financial, and discharge needs.
  • Screens all admissions to identify patients requiring complex case management and/or referrals to other healthcare team members.
  • Monitors caseload daily for variances to patient progression and collaborates with physician and/or care team to address.
  • Identify system barriers that contribute to variances in patient progression.
  • Develops/reviews/revises and individualizes the plan of care to address the patient's nursing care needs for optimum patient care outcomes. (Consultation and resources, nurses as teachers.)
  • Collaborates with providers and other clinical disciplines as appropriate.
  • Provides teaching specific to the patient.
  • Care plan reflects continuing care needs and/or referrals.
  • Implements interventions that are consistent with the established plan of care in a safe and appropriate manner (Professional Models of Care).
  • Is familiar with, and follows, nursing protocols and standards of care.
  • Uses current knowledge in nursing practice based on standards and research literature.
  • Performs technical skills competently and independently to assure patient safety (autonomy).
  • Able to competently perform technical skills independently.
  • Is accountable to learn new skills and equipment within areas' specifications and time frame.

  • Communicates to appropriate personnel regarding unresolved issues.
  • Works within the scope of practice as defined by Nurse Practice Act.
  • Fiscally accountable.
  • Acts professionally responsible for human and other resources.
  • Charges accurately for use of supplies and equipment according to policy.
  • Knowledgeable of length of stay goals for each DRG and communicates with the provider.
  • Partners with post-acute care providers to decrease readmissions.
  • Has knowledge of “Severity of Illness/Intensity of Service” criteria and collaborates with UR nurse regarding the appropriateness of admission, continued stay, and level of care.
  • Performs concurrent reviews for patients with commercial payers (Acute Rehab only).
  • Coordination of Care
  • Collaborates with physician, interdisciplinary team and patient/family to develop comprehensive, proactive discharge plans for all complex patients and oversees and ensures the development of discharge plans for all patients.
  • Establishes effective relationships with patient/family to set reasonable goals and expectations for acute care, and to facilitate a safe and satisfactory transition to home or alternative post-acute facilities.
  • Accurately documents in the medical record, and/or electronic system and completes forms in accordance with documentation policies.
  • Tracks and communicates pertinent results (i.e., diagnostics and consulting physician recommendations) in timely manner to physician(s) to expedite medical decision making.
  • Actively participates in interdisciplinary rounds and targets actionable issues affecting patient care and clinical progression.
  • Facilitates timely referrals to care team (i.e.: Social Work, Rehab, Wound Care, Diabetes Clinic, Palliative Care, etc.).
  • Coordinates interdisciplinary team actions to support the discharge plan, i.; consulting physician address needed medications, facility arrangements, physician approvals and orders for discharge Direct Medical Education (DME) orders.
  • Participates and/or facilitates patient/family conferences to address clinical management issues and discharge plan.
  • Teaches patient/family regarding complex diseases and/or discharge instructions.
  • Participates in Performance Improvement activities.
  • Assists with ongoing update/development of critical path and standing order sets.
  • Participates in hospital-wide committees and initiatives as assigned (i.e., pneumonia, COPD, CHF, CABG).

Education
  • Bachelors of Science in Nursing (BSN) from an accredited college of nursing required, or obtained within 5 years from date of hire for nurses with an Associate Degree in Nursing (ADN).
  • Successful completion of annual mandatory education requirements.
Experience
  • Minimum of two years of relevant clinical experience.
License/Certifications
  • Current valid Registered Nurse (RN) License, valid compact multi-state license, or a temporary permit while awaiting licensure required for work state.
  • Current American Heart Association or American Red Cross Basic Life Support (BLS) strongly preferred at time of hire, required within 3 months of hire.
  • Certification(s) in area of specialty preferred.
    • From National Certification Corporation (NCC) within 1 year of eligibility.
  • Case Management certification preferred.
