Your Health in South Carolina is seeking a Registered Nurse to join our urgent care family. The role involves service delivery to patients in homes and facilities, coordinating with providers and care teams to ensure continuity and quality of care.

The RN will participate in care coordination, telehealth visits, and collaboration with hospice and other health professionals, maintaining EMR accuracy and engaging in quality initiatives while supporting a broad care network.

Qualifications

• Must be a licensed nurse in good standing with the appropriate board.
• Minimum of three (3) years clinical experience preferred.
• Demonstrated ability to supervise and direct professional and administrative personnel.
• Ability to read and communicate effectively.
• Strong written and verbal skills.
• Basic computer knowledge.
• Ability to manage and demonstrate effective leadership skills.
• Good interpersonal and communication skills under all conditions.

Responsabilités

• Coordinate care with patients, families, and healthcare providers.
• Facilitate telehealth or tele-pharmacy visits and coordinate with hospice teams.
• Rounding with physicians in assisted living or skilled nursing facilities.
• Maintain current list of providers and update EMR with required information.
• Participate in coaching calls and quality improvement projects.
• Perform other duties as required in the discretion of SC House Calls.
