Job summary

Hackensack Meridian Health At Home seeks a compassionate nurse to deliver focused 1:1 care in patients' homes with a flexible schedule and supportive team. You’ll work in a trusted home health setting where you can make a meaningful impact in patients' daily lives.

Qualifications

• Associate's Degree in Nursing or Diploma Certificate in Nursing required.
• Minimum of six months RN experience or one year LPN experience, preferably in home care or hospice.
• Excellent written and verbal communication skills.
• Proficient with Microsoft Office and/or Google Suite.

Responsibilities

• Provide 1:1 patient care in patients' homes.
• Maintain accurate patient records and communications.
• Coordinate with care teams to ensure smooth home health services.

Education, Knowledge, Skills and Abilities Required:
