Job summary

Amedisys is seeking a Registered Nurse to support Hospice After Hour On-Call in Largo, MD. This role covers Monday - Thursday 5pm - 8am on-call shifts, providing initial assessments and ongoing care coordination to home-based patients.

The RN will collaborate with the clinical team, document progress, educate families, and perform on-call duties as assigned. Current MD RN license, CPR, and at least 1 year of nursing experience are required.

Qualifications

• Current RN license, state-specific.
• 1+ year nursing experience.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Develops and updates the plan of care with the care team.
• Communicates patient progress to the clinical manager and care team.
• Maintains patient care records with accuracy and timeliness.
• Records pain and symptom changes and outcomes.
• Educates families on patient condition and needs.
• Makes referrals to other hospice disciplines as needed.
• Fulfills on-call responsibilities and duties as assigned.
• Performs other duties as assigned.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Completes, coordinates and revises the total plan of care and maintains continuity of patient care by collaborating with appropriate staff.
• Regularly communicates patient progress to the clinical manager, physician and care team.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Educates families on patient condition, needs and care.
• Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Performs other duties as assigned.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• One+ year of nursing experience required
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
