Job summary

BAYADA Home Health Care, Inc. is seeking a Full Time Admissions Registered Nurse to join our home health visit team covering Delaware County and areas of Philadelphia. You will be an integral part of a multidisciplinary team delivering skilled nursing and rehabilitative care in patients’ homes.

The role requires independent clinical judgment, head-to-toe assessments, wound care, medication reconciliation, and timely documentation while coordinating urgent visits and supporting families.

Qualifications

• Current nursing license in good standing in Pennsylvania.
• Graduation from an accredited nursing program.

Responsibilities

• Perform head-to-toe assessments and identify changes in patient condition.
• Provide care for acute and post-acute conditions in the home.
• Triage and manage urgent clinical situations independently in the home setting.
• Conduct medication reconciliation, teaching, and safety monitoring.
• Perform comprehensive wound assessments, care, and documentation.

Make an Impact as a Registered Nurse at BAYADA Home Health Care

BAYADA Home Health Care is currently seeking a Full Time Admissions Registered Nurse (RN) to join our home health visit team to cover Delaware County and areas of Philadelphia. You will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to remain at home. Home health experience required.

How You’ll Make an Impact:

• Use strong head-to-toe assessment skills with the ability to identify subtle changes in patient condition
• Utilize your experience with acute and post-acute care (e.g., CHF, COPD, wound care, IV therapy, post-surgical care)
• Triage and manage urgent clinical situations independently in the home setting
• Perform medication reconciliation, teaching, and safety monitoring
• Perform comprehensive wound assessment, care, and documentation

Qualifications for a Registered BAYADA Nurse:

• A current nursing license in good standing in the state of Pennsylvania
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
• At least one year of current or recent hands-on medical experience within the past two years with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med surg, sub-acute care or rehab units (medical or skilled nursing settings).
• Previous home health experience is required.

• Competitive pay ranges with a weekly pay schedule

Pay: Weekly

Base Pay, depending on qualifications and subject to negotiations

$60-$73 per point
