Job summary

LiveWell Homecare Agency, LLC is seeking an RN to provide independent clinical care in a homecare setting. The nurse will develop care plans, manage contingencies, and delegate tasks to other care providers to ensure optimal patient outcomes.

The role includes administering medications, documenting patient histories, and educating patients and families about care and resources. A current RN license is required in the state of employment or a compact license.

Qualifications

• Bachelor of Science Degree in Nursing preferred.
• At least 1 year of relevant nursing experience preferred.
• Current RN license in the state of employment or a compact license.

• Deliver nursing care based on the nursing process: assessment, diagnosis, planning, intervention, and evaluation.
• Address patients’ psychological, emotional, cultural, and social needs in accordance with their practice level.
• Administer medications and treatments following protocols, monitor for side effects, and evaluate effectiveness.
• Document patient history, symptoms, medications, and care provided.
• Assess learning needs and educate patients, families, and caregivers, identifying issues and available resources.
