Southeastern Integrated Care Llc is seeking a Registered Nurse (RN) to join the ACTT team in Wilmington, NC. The RN will manage medications with the psychiatrist, monitor beneficiaries, build rapport, and participate in daily ACT team activities to support person-centered care.

This full-time role requires a NC nursing license or compact license, at least one year of experience with adults who have serious mental illness, and the ability to educate and coordinate with other providers.

Formación

• Minimum one year of experience working with adults with serious mental illness.
• Associate’s Degree of Nursing (RN) required.
• NC nursing license or compact license to practice in NC.

Responsabilidades

• Manages the medication system with the psychiatrist, administers and documents medication treatment.
• Screens and monitors beneficiaries for medical problems and side effects (including nursing assessments, AIMS).
• Builds rapport and engages with beneficiaries.
• Engages in health promotion, prevention and education activities.
• Develops strategies to maximize taking medications as prescribed with beneficiary and care team.
• Coordinates services with other medical providers.
• Educates the team on monitoring psychiatric symptoms and medication side-effects.
• Provides first responder crisis support to beneficiaries in crisis.
• Attends daily ACT team meetings and participates actively.
• Prepares oral medications for delivery and administers injectables as prescribed.
• Directs and supervises LPN staff.
• Documents all billable encounters in the EMR within 24 hours.

Registered Nurse ACTT

Clinical ACT Full Time Wilmington, NC, US

Summary:

The Nurse (RN) is part of the Assertive Community Treatment (ACT) team that provides person-centered services addressing a breadth of beneficiary’s needs, helping him or her achieve their personal goals.

Essential Duties and Responsibilities:

• Manages the medication system in conjunction with the psychiatrist, administers and documents medication treatment;


• Screens and monitors the beneficiary for medical problems and side effects (including nursing assessments, completion of AIMS);


• Build rapport and engagement with beneficiaries


• Engage in health promotion, prevention and education activities;


• If the beneficiary is in agreement, develop strategies to maximize taking medications as prescribed (e.g., reviewing home environment to find cues to remind beneficiary to take medications; work with beneficiary and psychiatric care provider to scale back the number of times medications are taking each day);


• Communicating and coordinating services with other medical providers;


• Educating the team in monitoring psychiatric symptoms and medication side-effects;


• Provide “first responder” crisis response to any beneficiaries experiencing a crisis;


• Attending daily ACT team meetings and actively participating;


• Preparing oral medications for delivery and administering injectables as prescribed.


• Direct and supervise LPN staff;


• Accurately documents all billable encounters into southeastern Integrated Care’s EMR (electronic medical record) system within 24 hours. Any corrections will be entered within 24 hours of being notified.


• Other duties as assigned.

Education/Experience

• Minimum one year of experience working with adults with serious mental illness;


• Associate’s Degree of Nursing for (RN)

Required Skills/Abilities

• Working knowledge of psychiatric medications;


• Ability to empathetically interact with beneficiaries and their families;


• Strong clinical skills;


• Operate independently or with minimal supervision.

Certificates, Licenses, Registrations

• Valid NC Nursing Licenseor Compact License that allows practice in NC;


• Copy of a valid, unrestricted NC Driver’s license, proof of insurance
