Ascension at Home is seeking a Home Health Registered Nurse (RN) to provide in-home patient care with a 7 on/7 off schedule in the Milwaukee area. This role emphasizes compassionate, patient-centered care and collaboration with an interdisciplinary team to manage complex cases.

Qualifications

• RN license in the state you work
• Two years of experience as a registered nurse, home health experience preferred
• Current driver’s license
• Current CPR and BCLS certification

• Support patient recovery and ability to stay at home by coordinating care and educating patients and families
• Case manage and provide clinical oversight to interdisciplinary team
• Deliver high-quality home health nursing care to patients in the Milwaukee area
