CareBuilders at Home in West Palm Beach, FL seeks a PRN Registered Nurse (RN) Case Manager to deliver comprehensive home-based case management. You will complete admissions, develop care plans, supervise caregivers, and coordinate with the healthcare team to ensure quality, compliant services.

Responsabilités

• Complete new client admissions, including conducting initial assessments, obtaining medical histories, and developing individualized care plans.
• Meet with families and clients to discuss the benefits of home care and address any questions or concerns they may have.
• Develop the initial care plan based on client needs and preferences
• Oversee caregivers' adherence to the care plan, including conducting regular visits to client homes to assess care quality and provide guidance and support to caregivers.
• Ensure all services provided are in compliance with state regulations and agency policies and procedures.
• Collaborate with other members of the healthcare team, including physicians, therapists, and social workers, to coordinate care and ensure continuity of services.
• Maintain accurate and up-to-date documentation of client care, including progress notes, care plans, and medication records.
• Provide education and support to clients and their families on health management, medication management, and safety practices.
• Participate in case conferences and team meetings to discuss client progress, identify areas for improvement, and develop strategies to enhance care delivery.
• Act as a liaison between clients, caregivers, and other healthcare professionals to facilitate communication and ensure the highest level of care and support.
• Coordinate and oversee the implementation of care plans, including coordinating with other healthcare professionals and community resources as needed.
• Provide ongoing monitoring and evaluation of client progress and make adjustments to care plans as necessary.
