Washington Square Nursing & Rehab in Warren, OH, a 83-bed skilled nursing facility, is seeking a compassionate Registered Nurse to join our team and provide high-quality patient care. You will be part of a warm, supportive environment focused on safety and resident well-being.

Responsibilities

• Provide direct patient care, including administering medications, performing treatments, and monitoring patient conditions

• Participate in multidisciplinary team meetings to discuss patient progress and develop strategies for improvement
• Maintain accurate and up-to-date patient records, including assessments, progress notes, and discharge summaries
• Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure seamless care coordination
• Provide emotional support and education to patients and families, addressing their physical, emotional, and psychological needs
