Responsibilities

• Administer medications, treatments, and skilled nursing procedures in home health setting.
• Monitor and document patient progress, vital signs, and changes in condition.
• Coordinate care with physicians, therapists, and other healthcare team members.
• Educate patients and families on disease management, medications, and home safety.
• Ensure compliance with state, federal, and agency regulations and documentation standards.
