Reach Project in Ithaca, NY is hiring a Community Health Registered Nurse to deliver compassionate, stigma-free care in a low-threshold harm reduction setting. You will perform nursing, social determinants of health assessments, and coordinate MOUD/PrEP/PEP and hepatitis C testing with autonomy.

Position Title: Community Health Registered Nurse
Community Health Registered nurses (CHRN) at REACH Medical, PLLC work to ensure that all patients and their family/support systems receive the highest quality health care. Our practice aims to consistently meet the goals of creating a safe, inclusive, professional, and friendly work environment for staff, patients, and clinical providers.
The primary duties of the CHRN at REACH Medical will be to carry out all nursing clinical, social determinants of health, and administrative tasks required to ensure great patient care occurs without stigma in our low threshold harm reduction medical practice. The CHRN will have much autonomy in terms of several of the harm reduction programs including patient centered recovery plans, medication for opiate use disorder (MOUD), PrEP, PEP, and Hep C testing and treatment options. The CHRN will therefore need to gain extensive knowledge in these areas of patient care. The CHRN will be responsible, along with all other staff, to be sure patients are scheduled for the appropriate visit type and transportation is scheduled, if necessary. The CHRN will ensure that the rooms are stocked and ready for patients to be seen. The CHRN will make sure patients are triaged, along with assuring patient intakes and screenings for social determinants of health, behavioral health, court case involvement, and food security have been done appropriately. The CHRN will ensure that the medical provider sees the patient in a timely manner and will also ensure that the discharge of the patient from the visit is done appropriately.
Use HIPAA compliant texting software assigned by the practice in order to communicate about prescriptions or other patient information with the patient.
