Job summary

Ohioans Home Healthcare in Toledo is seeking a Registered Nurse (RN) to provide in-home care across diverse, high-acuity cases. You will assess, plan and implement patient care, educate patients and families on disease processes, procedures and medications, and collaborate with Case Manager and clinicians to ensure continuity of care.

Qualifications

• One year of acute care experience required.
• Current RN state licensure (Ohio) required.
• CPR certification required.

Responsibilities

• Assess, plan and implement patient care in home health settings.
