Job summary

Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate comprehensive home health services. You will interact with multiple medical disciplines and perform the patient admission visit to determine eligibility for home care, then develop and follow a detailed plan of care in collaboration with the patient and family.

Qualifications

• Graduate of an accredited nursing program (diploma/ADN/BSN).
• Current state license as a Registered Nurse (RN).
• Current state Driver’s License required.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Interact with other medical disciplines as needed to provide professional care.
• Perform admission visit to the patient in the home and determine eligibility for home care services.
• Develop and follow the plan of care for each patient.
