Job summary

BST Family Hospice LLC is seeking a compassionate Registered Nurse Case Manager to join our hospice team in Woodruff, SC. The RN coordinates and provides individualized end-of-life care in assisted living and memory-care communities.

The ideal candidate is organized, clinically confident, and skilled at communicating with patients, families, facility staff, physicians, and the interdisciplinary hospice team.

Qualifications

• Current, unrestricted Registered Nurse license in South Carolina.
• Minimum of one year of professional nursing experience.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and automobile insurance.
• Ability to travel throughout the assigned service area.
• Strong clinical assessment, critical-thinking, and time-management skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Professional communication and documentation skills.
• Ability to respond calmly and appropriately during end-of-life situations.
• Basic proficiency with electronic medical records and mobile technology.

Responsibilities

• Complete comprehensive nursing assessments and develop plans of care.
• Provide skilled nursing visits based on patient needs and frequency.
• Monitor changes in condition and communicate with hospice physician and team.
• Manage pain, dyspnea, wounds, anxiety, agitation, secretions, and other symptoms.
• Coordinate medications, equipment, and services.
• Educate patients, families, and facility staff on disease progression and care.
• Provide emotional support and compassionate guidance.
• Complete admissions, recertifications, routine and PRN visits, and death visits.
• Maintain accurate electronic clinical documentation.
• Participate in interdisciplinary meetings and quality improvement activities.
• Collaborate with aides, social workers, chaplains, physicians, and facility staff.
