Job description

Position: Registered Nurse – Hospice

Location: South Bend, IN

Schedule: Full-Time | Monday–Friday | 9:00 AM – 5:00 PM

Setting: Community-Based Hospice (On-Site/Field Work)

As a Hospice RN, you will:

• Provide compassionate, patient-centered hospice nursing care in homes, facilities, or community settings
• Perform assessments, develop individualized care plans, and monitor patient progress
• Educate families on care needs, comfort measures, and available support
• Collaborate closely with the interdisciplinary hospice team
• Provide emotional support to patients and loved ones with warmth and respect

Candidate Requirements:

• Active RN license (Indiana)
• Minimum 2 years of hospice experience
• Reliable transportation for field visits
