Job summary

TEEMA GROUP is seeking a compassionate RN Case Manager in Carson, CA for a 13-week, full-time contract. The role blends clinic and field work, requiring an active CA RN license, BLS, and 2+ years of elder care experience.

You will assess patients, develop individualized care plans, educate families, and coordinate across care settings to prevent hospitalizations and improve outcomes.

Qualifications

• Active, unrestricted California RN license.
• Graduate of an accredited School of Nursing.
• Current Basic Life Support (BLS) certification.
• Minimum of 2 years of RN experience caring for frail or elderly adults.

Responsibilities

• Manage an assigned panel of patients and own clinical outcomes.
• Perform comprehensive nursing assessments in clinic, virtually, and at home.
• Develop, implement, and update Plans of Care with providers, patients, families, and caregivers.
• Educate patients and families on chronic illness management, medications, and prevention.
• Identify changes in patient condition and coordinate timely interventions with the team.
• Coordinate care across hospitals, SNFs, home health, hospice, and specialists.
• Facilitate transitions of care to reduce avoidable hospitalizations.
• Coordinate referrals for specialty care, labs, procedures, and transportation.
• Monitor hospice transitions and collaborate with hospice agencies.

What You'll Do

Patient Care & Care Coordination

Manage an assigned panel of patients while taking ownership of clinical outcomes and quality of care.

Perform comprehensive nursing assessments in the clinic, virtually, and during home visits.

Develop, implement, and update individualized Plans of Care in collaboration with providers, patients, families, and caregivers.

Educate patients and families on managing chronic illnesses, medications, disease prevention, and healthy living.

Identify changes in patient condition and coordinate timely interventions with the healthcare team.

Case Management

Coordinate care across hospitals, emergency departments, skilled nursing facilities, home health, hospice, and specialty providers.

Facilitate smooth transitions of care to reduce avoidable hospitalizations and improve patient outcomes.

Coordinate referrals for specialty care, laboratory services, procedures, personal care, transportation, vision, dental, and community resources.

Monitor hospice transitions and collaborate with hospice agencies when appropriate.

Clinical Support

Perform nursing procedures and direct patient care in both clinic and home settings.

Manage provider inbox tasks, medical orders, laboratory results, and follow-up care.

Communicate changes in patient condition to providers and interdisciplinary team members.

Maintain accurate, timely, and compliant electronic health record (EHR) documentation.

What We’re Looking For

Required Qualifications

Active, unrestricted California Registered Nurse (RN) license.

Graduate of an accredited School of Nursing.

Current Basic Life Support (BLS) certification.

Minimum of 2 years of RN experience caring for frail or elderly adults.

Strong clinical assessment and critical thinking skills.

Excellent communication, organizational, and multitasking abilities.

Ability to work independently while collaborating with an interdisciplinary healthcare team.

Comfortable traveling locally for home visits (2–5 times per week).
