Job summary

Entertimeonline is seeking a compassionate Home Health Registered Nurse (RN) to deliver skilled nursing care in patients' homes. You will assess, plan, implement, and coordinate care with the interdisciplinary team to improve health and independence.

The role requires Missouri RN licensure, CPR certification, and the ability to travel. New graduates are welcome, and a $5,000 sign-on bonus is offered along with competitive compensation and mileage reimbursement.

Qualifications

• Current, unrestricted RN license in Missouri.
• Graduate of an accredited nursing program.

• Current CPR certification required.
• Valid driver's license and reliable transportation.
• Ability to travel to patients' homes throughout the assigned service area.

Responsibilities

• Provide skilled nursing care to patients in their homes according to physician orders and the individualized plan of care.
• Perform comprehensive nursing assessments and regularly evaluate patient status.
• Develop, implement, and update individualized plans of care.
• Monitor patient progress and identify changes in condition requiring intervention.
• Administer medications and treatments as ordered.
• Perform wound care, medication management, injections, catheter care, and other skilled nursing procedures.
• Coordinate care with physicians, therapists, social workers, aides, and other healthcare professionals.
• Participate in interdisciplinary care planning and case conferences.
• Complete all required clinical documentation accurately and timely.
• Maintain compliance with Medicare, Medicaid, state, federal, agency, and accreditation requirements.
• Maintain patient confidentiality and HIPAA compliance.
• Follow infection-control and patient-safety procedures.
• Provide on-call services as assigned.
• Maintain effective communication with patients, families, caregivers, physicians, and agency staff.
• Promote a positive patient experience and represent the organization professionally.

We are seeking a compassionate, dependable, and skilled Registered Nurse (RN) to join our Home Health team. The Home Health RN provides skilled nursing care to patients in their homes, helping individuals maintain their health, independence, and quality of life while avoiding unnecessary hospitalizations.

The RN will assess patients, develop and implement individualized plans of care, provide skilled nursing services, educate patients and caregivers, and collaborate with physicians and the interdisciplinary care team.

Provide skilled nursing care to patients in their homes according to physician orders and the individualized plan of care.

Perform comprehensive nursing assessments and regularly evaluate patient status.

Develop, implement, and update individualized plans of care.

Monitor patient progress and identify changes in condition requiring intervention.

Communicate significant changes in patient condition to the physician and appropriate members of the care team.

Administer medications and treatments as ordered.

Perform wound care, medication management, injections, catheter care, and other skilled nursing procedures as appropriate to the patient's needs.

Monitor vital signs, symptoms, pain levels, and other clinical indicators.

Provide patient and caregiver education regarding medications, disease processes, treatment plans, safety, nutrition, and self-care.

Coordinate care with physicians, therapists, social workers, aides, and other healthcare professionals.

Participate in interdisciplinary care planning and case conferences.

Complete all required clinical documentation accurately and timely .

Maintain compliance with Medicare, Medicaid, state, federal, agency, and accreditation requirements.
