Ambercare Hospice in Belen, NM is seeking a compassionate Registered Nurse Case Manager to provide expert clinical care and coordinate patient plans of care across the interdisciplinary team.

You will assess patients and families, manage admissions, plan, implement, and evaluate care, and support families through life events while promoting comfort and dignity. Competitive benefits and growth opportunities are included.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess CPR and First Aid certification.
• Valid driver's license and proof of insurance.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation, and evaluation.
• Obtain data from patient factors and initiate intervention by the interdisciplinary team.
• Initiate communication with attending physician and other agencies to coordinate care.
• Provide regular visits to reassess patient needs and update the plan of care.
• Perform clinical pronouncement of death and support the family.
• Maintain accurate patient records and care plans; supervise care by Home Health Aide as required.

What You'll Do

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines

Qualifications

• Graduate from an accredited registered nursing program
• Licensed as a registered nurse in the state of practice
• Must possess current CPR, First Aid Certification
• Must be knowledgeable of quality assessment and assurance procedures
• Valid driver's license and proof of insurance is required
