Job summary

Qualifications

  • Current NYS RN license is required.

Responsibilities

  • Assess patients and respond to changes in condition.
  • Perform nursing assessments and health history reviews.
  • Provide triage via telephone and in-person per protocols.

Job description

For more than 40 years, Morris Heights Health Center (MHHC) has provided comprehensive, affordable, and culturally responsive healthcare to communities throughout the Bronx and Brooklyn. As a Federally Qualified Health Center, MHHC delivers integrated primary care, specialty care, behavioral health, dental, school-based health, women’s health, and supportive services to patients across the lifespan.

We are seeking a compassionate and clinically skilled Registered Nurse to join our interdisciplinary ambulatory care team. This is an opportunity to provide high-quality nursing care while helping patients manage their health within a community-based setting.

Position Summary

The Registered Nurse provides direct clinical support, patient education, care coordination, and nursing oversight in collaboration with physicians, advanced practice providers, licensed practical nurses, medical assistants, and other members of the care team.

The RN supports safe and efficient clinical operations, promotes continuity of care, and helps ensure that patients receive timely, evidence-based, and culturally responsive services.

Key Responsibilities
  • Assess patients’ clinical needs and respond appropriately to changes in condition.
  • Perform nursing assessments, including vital signs, medication reconciliation, health histories, and evaluation of presenting symptoms.
  • Provide telephone and in-person clinical triage using established nursing protocols.
  • Administer medications, immunizations, and treatments in accordance with provider orders and organizational policy.
  • Educate patients and families regarding diagnoses, medications, treatment plans, preventive care, and chronic disease self-management.
  • Coordinate care with providers, specialists, hospitals, pharmacies, laboratories, and community-based organizations.
  • Conduct follow-up outreach for abnormal results, hospital and emergency department discharges, missed appointments, and identified care gaps.
  • Support chronic disease management initiatives involving diabetes, hypertension, asthma, cardiovascular disease, and other prevalent conditions.
  • Review and address clinical messages, test results, referrals, and patient-care tasks within the electronic health record.
  • Assist with emergency response and ensure that emergency medications, equipment, and supplies are properly maintained.
  • Promote infection prevention, medication safety, patient identification, and other clinical quality and patient-safety standards.
  • Participate in care-team huddles, quality-improvement activities, population health initiatives, and regulatory-readiness efforts.
  • Provide appropriate clinical guidance and support to nursing and clinical support staff within the RN’s scope of practice.
  • Maintain accurate, timely, and complete clinical documentation.
  • Protect patient confidentiality and comply with HIPAA, infection-control requirements, and all applicable federal, state, and organizational standards.
  • Perform other nursing responsibilities consistent with the needs of the assigned department or health center.
Required Qualifications
  • Current and unrestricted New York State Registered Professional Nurse license.
  • Graduate of an accredited nursing program.
  • Current Basic Life Support certification through the American Heart Association or another approved provider.
  • Strong clinical assessment, triage, documentation, and patient-education skills.
  • Ability to work effectively within an interdisciplinary healthcare team.
  • Excellent communication, organizational, and problem-solving skills.
  • Commitment to serving diverse and medically underserved communities.
  • Ability to use an electronic health record and other clinical technology platforms.
Preferred Qualifications
  • Bachelor of Science in Nursing.
  • At least one year of RN experience in an ambulatory care, primary care, community health, hospital, or related clinical setting.
  • Experience with telephone triage, care management, population health, or chronic disease management.
  • Experience working within a Federally Qualified Health Center or similar community-based healthcare organization.
  • Bilingual ability, particularly English/Spanish, is highly desirable.

Registered Nurses receive 20 days of paid vacation annually, in accordance with the collective bargaining agreement.
