Job summary

The Bernard Health Institute is seeking a Registered Nurse Supervisor to lead the planning and management of patient care plans in a home care setting. You will be an RN who emphasizes detail and compassionate, individualized care, reviewing intake forms to assign direct care staff and orienting new staff to agency policies.

Applicants should demonstrate current nursing practice knowledge and strong problem-solving abilities, with an emphasis on patient advocacy and coordination with care teams.

Qualifications

• Licensed RN in the state of employment.
• Experience as RN preferred, geriatric or home care experience preferred.
• Strong interpersonal and care-planning skills.
• Valid driver’s license and CPR certification.

Responsibilities

• Administer medications and monitor patients for side effects or reactions.
• Document changes in patient conditions and report to supervisor.

Job Summary

We are seeking a Registered Nurse Supervisor to join our team. In this role, you will primarily be creating and managing a plan of care for individual patients. You are a Registered Nurse (RN) who operates with great attention to detail and a big heart. You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest. Identifies and evaluates client needs and appropriateness of home care by reviewing the initial intake screening form before assignment of direct care staff . Assists with the orientation of new professional staff to familiarize them with Agency policies and procedures.
To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems.

Responsibilities

• First aid/CPR certification required
