Registered Nurse (RN)

Provide resident-centered clinical care in skilled nursing, rehabilitation, post-acute recovery, long-term care, and memory care settings. Perform patient assessments, administer medications and treatments, and develop and coordinate care plans in collaboration with an interdisciplinary team.

• Monitor residents for changes in condition and respond with appropriate nursing interventions
• Supervise and support Certified Nursing Assistants (CNAs)
• Document assessments, care, and outcomes in the Electronic Medical Record (EMR)
• Communicate effectively with families and healthcare providers
• Coordinate care across services to support recovery, safety, and quality of life

Qualifications & Skills

• Current RN licensure and strong clinical assessment and medication administration skills
• Ability to collaborate with interdisciplinary teams and manage resident-centered care
• Strong communication skills and attention to documentation requirements
• Comfort working in a fast-paced, 24/7 clinical environment
