Job summary

Home Health Care 2000 is seeking an experienced Registered Nurse (RN) to perform case management for home health services under a physician's plan of care. The role supports coordination of patient care, management of home health staff, and timely documentation in EMR systems.

The candidate should have 1+ year in medical-surgical nursing, familiarity with Medicaid/Medicare rules, and a current Louisiana RN license. Part-time position with benefits is available.

Qualifications

• RN with at least 1 year in medical-surgical adult health nursing
• Home health experience preferred but not required
• Experience managing care plans and coordinating multidisciplinary teams
• Knowledge of Medicaid/Medicare regulations and Joint Commission standards is helpful

Job Summary PART TIME POSITIONS AVAILABLE

A qualified Registered Nurse (RN) candidate to perform the responsibilities of case management of home health care services to our patients based on a physician's plan of care.

Responsibilities and Duties

• Coordinate multi disciplinary activities related to the patient's care plan
• Manage home health care personnel
• Process and manage authorizations for patient care services
• Document and chart data using electronic medical record system
• Complete timely documentation according to the rules of participation in medical service plans

Qualifications and Skills

• One or more years experience in medical-surgical adult health nursing
• Experience in home health preferred but not required
• Must be experienced in managing care plans
• Experience with coordination of multi disciplinary activities and management of home health care personnel
• Self-starter with excellent organizational and writing skills
• Experience in current Medicaid, Medicare Regulations and Joint Commission standards home health standards helpful
• Has maintained a current and active registered nurse (RN) license in the State of Louisiana
• Experienced in electronic documentation
