Job summary

Gentiva is seeking a compassionate Registered Nurse to provide home-based clinical care to patients and families. You will perform comprehensive assessments, develop and update individualized care plans, administer medications and treatments, and educate patients and caregivers on disease management and safety in the home.

You will coordinate care with an interdisciplinary team, ensuring dignity, comfort, and quality outcomes.

Qualifications

• Must hold an active Registered Nurse license (RN).
• Experience delivering home health care and patient education preferred.
• Compassionate, patient-centered approach with strong communication skills.
