Transitions in Erlanger, KY, is seeking a Registered Nurse to train staff in MOUD and to coordinate patient care across residential and outpatient settings.
You will collaborate with MAT/MOUD providers, participate in case conferences, and support patients during transitions while ensuring safety and adherence to treatment plans.
Plays a crucial role in training and mentoring healthcare staff regarding substance use disorders and the use of MOUD as a treatment mode. They are actively involved in patient care assessment, planning, implementation, and evaluation with a specific focus on the use of MOUD for substance use disorder-related healthcare. Additionally, they collaborate with community partners for strategizing care plans for patients with substance use disorder needs. They actively participate in discharge planning by addressing health requirements within other facilities and upon discharge into the community. Effective collaboration and communication with various disciplines across the community are vital aspects of this role.
Registered Nurse with current RN license in State of Kentucky. CPR certification required. Two years nursing experience with demonstrated ability to inspire others to excellence. At least one year of chemical dependency experience is desired. Additional nursing experience in behavioral health, extended healthcare and acute healthcare preferred.
