Job summary

Healthy Living at Home - El Centro, LLC is seeking a professional Registered Nurse Case Manager to provide home health care to Imperial County residents. You will assess, plan, and coordinate patient care, educating patients and families in their homes and communities.

The role requires an unencumbered RN license, CPR certification, and strong communication skills. Travel within the El Centro area and bilingual English/Spanish preferred are advantageous.

Qualifications

• Holds an unencumbered Registered Nurse (RN) license.
• Graduate of National League for Nursing (NLN) accredited school of nursing.
• Minimum of one (1) year Registered Nurse experience.
• Minimum of one (1) year Home Health experience.
• Possess and maintain a current CPR certification.
• Must be a licensed driver with an automobile that is insured and in good working order.
• Be self-directed and able to work with minimal supervision.
• Demonstrates strong observation, problem-solving, verbal and written communication skills.
• Able to organize and prioritize workload independently.
• Able to lift and transfer patients as needed.
• Bilingual English/Spanish preferred.

Responsibilities

• Completes an initial assessment of patient and family to determine home care needs.
• Provides a complete physical assessment and history of current and past illnesses.
• Regularly reevaluates patient nursing needs and revises plans as needed.
• Develops and initiates the plan of care with patient/family involvement.
• Initiates preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed.
• Counsels patient and family in meeting nursing and related needs.
• Identifies discharge planning needs and coordinates prior to discharge.
• Acts as Case Manager coordinating care for the assigned caseload.

Healthy Living at Home - El Centro, LLC is in search for a professional and caring Registered Nurse Case Manager to provide case management, assessment, evaluation, direction and care to our Home Health patients in the Imperial County - El Centro metropolitan area.Position OverviewThe Registered Nurse Case Manager plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Essential Job Functions

• The Registered Nurse Case Manager completes an initial assessment of patient and family to determine home care needs.
• Provides a complete physical assessment and history of current and previous illness(es).
• Regularly re-evaluates patient nursing needs.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician.
• Counsels the patient and family in meeting nursing and related needs.
• Provides health care instructions to the patient as appropriate per assessment and plan of care.
• Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
• Acts as Case Manager when assigned by Clinical Supervisor/Nursing Supervisor and assumes responsibility to coordinate patient care for assigned caseload.

Basic Qualifications

• Holds unencumbered Registered Nurse (RN) License
• Graduate of National League for Nursing (NLN) accredited school of nursing
• Minimum of one (1) year Registered Nurse experience
• Minimum of one (1) year Home Health experience
• Possess and maintain a current CPR certification
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Be self-directed and able to work with minimal supervision
• Demonstrates excellent observation, problem-solving, verbal and written communication skills; nursing skills per competency checklist
• Show ability to organize and prioritize workload independently
• Prolonged or considerable walking or standing. Able to lift, position, or transfer patients
• Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching.
• Visual acuity and hearing to perform required nursing skills.
• Bilingual English/Spanish Preferred

Physical RequirementsTypical functions of this role include standing, sitting and walking a majority of the time. There are occasions when kneeling, stooping, bending or reaching may be necessary.The employee must be able to lift and/or move up to 10 lbs on a regular basis. There may be occasions in which the employee must be able to lift or move up to 40 lbs.Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
