Job summary

The GreenFields Continuing Care Community in Lancaster, NY is seeking a compassionate RN Staff Nurse to provide resident-centered care in our skilled nursing community. You will assess residents' needs, administer medications, monitor condition, and collaborate with physicians and the care team to ensure the highest quality of care.

The ideal candidate holds a NYS RN license and a 2-year degree, with BLS/CPR certification and experience in geriatric or long-term care.

Qualifications

Responsibilities

• Provide compassionate, resident-centered nursing care in accordance with physician orders, nursing standards, and facility policies.
• Receive and provide thorough shift reports to ensure continuity of resident care.
• Assess, monitor, and document residents' physical, mental, and emotional status, reporting changes promptly to supervisor and physician.
• Administer medications, treatments, and interventions safely and accurately while monitoring resident responses.
• Maintain accurate and timely clinical documentation, including medication administration, treatments, assessments, care plans, and incident reports.
• Coordinate and supervise the delivery of care by CNAs and other nursing staff, providing guidance and support.
• Collaborate with physicians, rehabilitation staff, and the interdisciplinary care team to develop and update individualized resident care plans.
• Admit new residents, complete nursing assessments, and assist with developing individualized plans of care.
• Respond promptly to resident call lights with professionalism, compassion, and respect.
• Assist with resident meals, nourishment programs, and other daily care activities as needed.
• Participate in the orientation and mentoring of new nursing staff and support a collaborative team environment.
• Serve as Charge Nurse or assume leadership responsibilities when assigned.

Position Summary

We are seeking a compassionate and dedicated Registered Nurse (RN) to join our team and provide exceptional, resident-centered care in our skilled nursing community. Key Responsibilities

• Provide compassionate, resident-centered nursing care in accordance with physician orders, nursing standards, and facility policies.
• Receive and provide thorough shift reports to ensure continuity of resident care.
• Assess, monitor, and document residents' physical, mental, and emotional status, reporting changes in condition promptly to the appropriate supervisor and physician.
• Administer medications, treatments, and therapeutic interventions safely and accurately while monitoring resident responses.
• Maintain accurate and timely clinical documentation, including medication administration, treatments, assessments, care plans, incident reports, and other required records.
• Coordinate and supervise the delivery of care by Certified Nursing Assistants (CNAs) and other nursing support staff, providing guidance and support throughout the shift.
• Collaborate with physicians, rehabilitation staff, and the interdisciplinary care team to develop, implement, and update individualized resident care plans.
• Admit new residents, complete nursing assessments, and assist with developing individualized plans of care.
• Respond promptly to resident call lights and address resident needs with professionalism, compassion, and respect.
• Accompany physicians during rounds, communicate resident updates, and accurately transcribe physician orders as needed.
• Monitor inventory and ensure the availability of medications, treatment supplies, equipment, and other nursing resources.
• Promote resident safety by following infection prevention practices, medication safety standards, emergency procedures, and facility policies.
• Assist with resident meals, nourishment programs, and other daily care activities as needed.
• Participate in the orientation and mentoring of new nursing staff and support a collaborative team environment.
• Serve as Charge Nurse or assume leadership responsibilities when assigned, ensuring efficient unit operations and quality resident care.
• Participate in quality improvement initiatives, mandatory education, and ongoing professional development.
• Maintain resident confidentiality and comply with all HIPAA, regulatory, and facility requirements.
• Perform other related duties as assigned to support the nursing department and provide exceptional resident care.

PHYSICAL AND SENSORY REQUIREMENTS

• Must be able to stand and walk for extended periods throughout the workday.

• Must be able to frequently reach, bend, stoop, kneel, crouch, and perform other physical movements associated with resident care.
• Must possess adequate vision and hearing, or use appropriate adaptive devices, to perform the essential functions of the position.
• Must be able to cope with the mental and emotional demands of providing nursing care in a fast-paced healthcare environment.
• Must meet the general health requirements established by facility policies.
• May be required to assist with emergency procedures, including resident evacuation, as needed.

Working Conditions

• Works in a healthcare environment with regular interaction with residents, families, physicians, employees, and interdisciplinary team members.
• May work in resident rooms, nursing units, treatment areas, and other designated facility locations.
• Subject to frequent interruptions and changing priorities throughout the workday.
