Qualifications

• Active RN license.
• Graduate of an accredited nursing program.
• At least 1 year of nursing experience (preferred).
• Current CPR certification.
• Ability to work independently and manage time effectively.
• Strong clinical judgment, communication, and organizational skills.

Responsibilities

• Provide skilled nursing care in patients' homes per physician orders and care plans.
• Coordinate care and communicate changes with physicians and team members.
• Educate patients and families on care and health management.
• Maintain accurate, timely clinical documentation (including OASIS).
• Supervise LPNs, CNAs, and HHAs per regulatory guidelines.
• Support patients’ rehabilitative and preventive care needs.

As a Registered Nurse (RN), you will provide skilled nursing care to patients in their homes, working under physician orders and care plans, while collaborating with a dedicated clinical team.

• Broomfield & surrounding communities
• Deliver skilled nursing care based on individualized care plans
• Conduct initial patient assessments and ongoing evaluations
• Coordinate care and communicate changes with physicians and team members
• Educate patients and families on care and health management
• Maintain accurate, timely clinical documentation (including OASIS)
• Participate in care planning, case conferences, and quality improvement efforts
• Supervise LPNs, CNAs, and HHAs per regulatory guidelines
• Support patients’ rehabilitative and preventive care needs
