Job summary

Palms-Home-Care is seeking dedicated Home Health RNs for full-time and PRN positions in Florida. You will perform patient assessments, develop care plans, and coordinate interprofessional care to support patient health outcomes. Travel between patient homes is required, with supervision of LPNs and collaboration with physicians and therapists.

Ideal candidates hold a Florida RN license with 1+ year home health experience, strong clinical skills, and proficiency with EHR systems such as Kinnser.

Qualifications

• Current Registered Nurse (RN) license in Florida
• At least 1 year of home health nursing experience
• Excellent knowledge of nursing care methods and procedures
• In-depth knowledge of health and safety guidelines and procedures
• Team player with excellent communication and interpersonal skills
• Outstanding organizational and multi-tasking skills
• Familiar with EHR such as Kinnser
• Ability to work independently, manage a daily schedule, and travel between patient homes

Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans in collaboration with the care team.

• Coordinate care by making appropriate referrals or documenting clinical rationale when referrals are not indicated.

• Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.
• Supervise and support LPNs to ensure high standards of care.
• Perform additional duties as needed to support patients and team operations.
